I give, devise, and bequeath to United Way of Central Jersey, Inc., Tax ID 22-1520408, all [or state the fraction or percentage] of the rest, residue, and remainder of my estate, both real and personal, to be permanently endowed in [a general endowment fund or a named endowment fund], the annual payouts of which shall be contributed to the United Way Community Campaign.
